&#60;p&#62;BACKGROUND: Cardiac computed tomographic angiography (CTA) provides for accurate noninvasive diagnosis of coronary artery disease (CAD). OBJECTIVES: We analyzed the clinical outcomes over 40 months in patients with and without CAD as determined by CTA in an outpatient setting. METHODS: Consecutive symptomatic patients (n = 493; mean age, 58 +/- 15 years; 70% men) with an intermediate likelihood of CAD referred for outpatient CTA evaluation were prospectively followed for a mean of 40 +/- 9 months. RESULTS: Results of CTA included as normal (defined as normal coronary lumen), found in 32% (157), nonobstructive disease (&#38;lt;50% luminal stenosis) in 41% (204), obstructive disease (&#38;gt;or=50% luminal stenosis) in 19% (93). Eight percent (n = 39) had &#38;gt;or=1 major nondiagnostic coronary artery segment. Follow-up identified 21 patients with myocardial infarction (MI) in the significant obstructive CAD and nondiagnostic group. No patients with either normal coronary arteries or nonobstructive disease experienced an MI during follow-up. The 40-month event-free survival was 100% for both the normal and nonobstructive disease groups, 97.5% for the nondiagnostic study group, and 79% for the group with obstructive CAD. After adjustment for age, sex, diabetes mellitus, hypertension, hypercholesterolemia, and baseline coronary artery calcium (CAC), a stepwise multivariable model (Cox regression) showed that obstructive CAD was an independent predictor of cardiac events and had significant incremental value over clinical risk factors and CAC (HR = 16.6; 95% CI, 4.9-55.2; P = 0.0001). CONCLUSION: In symptomatic patients with an intermediate likelihood of CAD referred for CTA, normal coronary arteries or nonobstructive CAD portends an excellent prognosis. The finding of obstructive CAD identifies patients at higher risk of subsequent MI, independent of cardiovascular risk factors and coronary artery calcium.&#60;/p&#62;

&#60;p&#62;BACKGROUND: The American College of Cardiology Foundation/American Society of Nuclear Cardiology appropriateness criteria document assigns single photon emission computed tomography myocardial perfusion imaging (SPECT MPI) a rating of uncertain for detection and risk assessment of coronary artery disease (CAD) in asymptomatic patients at moderate risk. METHODS AND RESULTS: The nuclear cardiology database was used to identify 260 asymptomatic patients (67 +/- 8 years, 72% men) without known CAD who were at moderate CAD risk according to the Framingham risk score. SPECT MPI images were categorized using the summed stress score (SSS). Mean follow-up 9.9 +/- 3.0 years. Abnormal SPECT MPI scans were present in 142 patients (55%). By SSS categories, SPECT scans were low-risk in 67%, intermediate-risk in 20%, and high-risk in 13% of patients. Overall survival at 10 years was 79%, significantly better than the age- and gender-matched Minnesota general population (P &#38;lt; 0.001). Survival was 60% for patients with high-risk scans (95% CI 45-80%), 79% with intermediate-risk scans (95% CI 69-91%), and 83% with low-risk scans (95% CI 77-88%) (P = 0.03), including 84% (95% CI 77-91%) with normal scans. CONCLUSIONS: In this retrospectively identified group of asymptomatic patients at moderate CAD risk, stress SPECT MPI was effective for the detection and risk stratification of CAD. Average annual mortality was 4.0% in patients with high-risk scans vs 1.6% in patients with normal scans.&#60;/p&#62;

METHODS: A total of 17 patients with atherosclerotic abdominal aortic aneurysm underwent dynamic abdominal PET/CT using 2-min frames between 45 and 53, 57 and 65, 115 and 123, and 175 and 183 min after injection of (18)F-FDG. For each period of dynamic imaging, vessel wall and lumen uptake were measured using the maximum standardized uptake value (SUVmax) and target-to-background ratio (TBR). RESULTS: No significant difference in TBR across all time points (repeated measures ANOVA, P = 0.206) was observed, despite a significant difference in aortic wall and lumen uptake with time (repeated measures ANOVA, P = 0.02 and P &#38;lt; 0.001, respectively). There was no significant difference between aortic wall uptake at 60 min (SUVmax, 2.15 +/- 0.11 SE) and 180 min (SUVmax, 1.99 +/- 0.18 SE) (paired t test, P = 0.367). There was a significant difference in lumen uptake at 60 min (SUVmax, 2.4 +/- 0.11 SE) and 180 min (SUVmax, 1.7 +/- 0.1 SE) (paired t test, P = 0.001). There was no significant difference in TBR between 60 min (0.91 +/- 0.03) and 180 min (1.01 +/- 0.06 SE) (paired t test, P = 0.131). With increasing delayed imaging, there was increasing variability (SE) in the SUVmax for the aortic wall and TBRs. CONCLUSION: There was no significant advantage in imaging at 3 h over 1 h after (18)F-FDG injection.&#60;/p&#62;
